  • Title: Is Ponderal Index a Better Predictor for Infant Morbidity than Large for Gestational Age (Lga) in Women with Type 1 Diabetes?

Abstract

Aim: To investigate if ponderal index (PI) is associated with glycaemic control, assessed by

continuous glucose monitoring (CGM), in pregnant women with type 1 diabetes, and if PI is a better indicator than large-for-gestational-age (LGA) for predicting infant morbidity.

Methods: Associations between CGM-variables and PI, as well as associations between PI/LGA, hypoglycaemia, asphyxia and composite morbidity were analysed with crude and adjusted logistic regressions, in separate analysis. Overlaps between PI and LGA were analysed using chai-square test.

Results: In the first trimester, mean HbA1c was significantly higher in women delivering high PI infants (p=0.023). In the second and third trimesters, time in target (TIT) was associated with a significantly lower risk of delivering an infant with high PI, p=0.0,017 and p=0.004 respectively. The risk of delivering an infant with high PI was significantly higher with more

time above target (TAT) in the second and third trimester, p=0.036and p=0.007. Mean glucose and standard deviation (SD) were also significantly associated to an increased risk of high PI in third trimester, p=0.04 and p= 0.046, respectively. No associations were found between PI and infant morbidity. LGA was associated with a 4-fold increased risk of infant morbidity and a doubled risk of neonatal hypoglycaemia.

Conclusions: Poor glycemic control in all trimesters increased the risk of having an infant with high PI, however LGA is better than PI for predicting infant morbidity.
